Request for Transcript of Tax Return (4506-T form)
Here are sections to remember when completing the 4506-T form:
PAGE 1:
1. The borrower signs and dates near the bottom of the page.
2. If the borrower is married, be sure the spouse signs and dates below the borrower’s signature.
PAGE 2:
Sometimes a lender will print initial lines at the bottom of the page, or give instruction on the file to add them to the document. If this is the case, please make sure the borrower initials where required. The borrower’s spouse will also initial if the borrower is married.

This is good. I like showing where you need to be aware of who signs where and that initials are
needed to be here.
If in the loan package there are separate 4506-T why do they have to sign each others? I do but am curious the reason. Thanks, Maureen
The spouse signs when it’s a joint return.
I’m unsure why the IRS requires separate forms, but with a little research you can probably find out.
So if there is a 4506 for each borrower, they need to sign each others forms as spouse also? I have never had borrowers sign this way and have never been called on it.
If the spouses file jointly, then have each spouse sign their partner’s form. Some lenders are more lenient than others, since according to the IRS they’ll furnish the copy as long as one of the spouses signs the document. While other lenders require the form completed as the form suggests. This is the safer way to cover your bases for all lenders.
If line 6 & 9 are blank, the form says “Do not Sign” I don’t see many that are completed. How is this handled. I have had some B’s refuse to sign.
Reassure the borrower that the client normally fills in the info once they receive the package after the signing. If the borrower still refuses to sign until it’s complete, we’d recommend the borrower call their Loan Officer for instruction.
I need to get transcripts dated back to 2005. How specific do I need to be when submitting the dates? Do I need to give the EXACT month, date, and year? Do I just list the month and the year? Or will it be ok if I just leave the year?
Anyone’s assistance would be greatly appreciated!
Enter the ending year or period date in the format suggested. If you need the transcript for 2005, enter 12/31/2005.
can you use a PO Box address for the taxpayer?
If the client has a PO Box that is fine to put in line 3, but they are required to fill in line 4 as their previous address, the one that was listed on their last tax return.
Tax Years should always be listed as 12/31/2007 or whatever fiscal year the clien uses to file their tax returns.
TIP: IRS must receive Form 4506-T within 60 days of the date signed by the taxpayer or it will be rejected. So as long as the client signs & date’s the form you can advise them that it is only good for 60 days.
